FIELD INSPECTIONS

Verify Production Company Information• Name, Address, Phone, Title2.Verify Location of Production• Name of Facility, Address, City, State3. Verify Production Type• Options List (Stage, Motion Picture,Television, Rehearsal, Other)4. Verify Contact Details•Name, title and phone# of the Contact1. Show List of Minors, Let InvestigatorSelect One by OneFor each minor, show• Display Name, Address, DOB,Guardians Name and Tel #• Cannot Modify, if wrong add as new(submitted as Inspection Data)2. Capture Production Details per MinorCapture Production Details for this Minor• Get Start and End times• Verify Production Details• Verify Location Details• If already Captured press Duplicate3. Identify Minor and Start Questionnaire• Q1 , Q2 Q3, Props etc(Options like -> Yes, No, Skip )Can take picture or Record Video4. Identify Parent/Guardian and Start Questionnaire• Q1 , Q2 Q3, Props etc(Options like -> Yes, No, Skip )Can take picture or Record Video5. • Comments/Other Text• Take Pictures / Video / Audio Notes• Common and Final questions for each Minor.6.Display Summary of Minor and Verify.Inspector completes Data Verificationmodule.App updates iOTA in the backgroundwith Verification module results andsuggested changes (if any).OKInspector completes Minors module(for Theatrical Inspection) or Equipment module(for Boilers inspection).App updates iOTA in the backgroundwith the inspection results, comments andMEDIA attachments (if any).OKInspector clicks "Submit Inspection"button.App updates backend that theInspection has completed.System notifies the Manager's UI orapp when an Inspection has finished.System e-mails the Managerwith an Inspection report.System e-mails the Inspectorwith an Inspection report.OK[opt]Manager reviews the informationreceived and can reassign the inspectionto the same or different inspector(Label (1) in "START ASSIGNMENT")[opt]Manager can PRINT / EMAIL / FAXthe issued permit. iOTA is updatedwith the status of application.iOTA Portal helps Manager send out afinal email to recipients aboutthe freshly issued permit.
